Job Description

Statement of Purpose: This position is responsible for social service support to enhance and maintain the quality of life of our residents.  Assists the Health Care Center Administrator in complying with current applicable federal and state laws, and facility policy and procedures. This position is exempt from overtime pay.

Major Tasks, Duties, and Responsibilities:

  • Serve as the resident advocate assuring that resident’s rights are upheld (such as the right to fair and equitable treatment, self-determination, individuality, privacy, property, and civil rights, including the right to file a complaint).
  • Serves as liaison between residents, families, and community agencies.
  • Establishes working relationships with community agencies and service providers.
  • Assesses residents for service needs and refers to available Campus services.
  • Refers and provides information for the residents and/or their relatives to community service providers, up to and including Hospice referrals.
  • Coordinates discharge to appropriate level of care (such as discharging off skilled services notification letter).
  • Follows through with established expectations and keeps residents, families and coworkers informed of progress.
  • Maintains resident social service files; keeps progress notes.
  • Facilities and coordinates care plan meetings with interdisciplinary team.
  • Completes required documentation such as psychosocial assessments, MDS, Care Plan, RAPS, etc.
  • Coordinates room moves.
  • Assists residents, families, and visitors with the concern/compliment process.
  • Ensures residents’ Advance Directives preferences are properly documented.
  • Assists residents and families with financial entitlement programs.
  • Establishes working relationships with medical case managers and ombudsman.
  • Makes reports to Adult Protective Services as required.
  • Consistently maintains complete confidentiality of all medical, financial, employee, computer or other sensitive material which may jeopardize the privacy of others.
  • Demonstrates excellent customer service in all interactions by conducting oneself in a cooperative, courteous, positive, and professional manner. Displays sensitivity and awareness of others. 
  • Provides psychosocial interventions.
  • Facilitates Neighborhood, Core Team and Resident Council meetings as applicable.
  • Conducts Resident Right’s Section of New Employee Orientation and Celebration of Services.
  • Performs other duties as requested or assigned.

Education and Experience:

  • BSW from accredited school of social services or Bachelors or Master’s Degree in a related field from an accredited college or university
  • One year experience in healthcare setting, preferably long-term care.
  • Working knowledge of State and Federal rules, regulations, and laws as they pertain to the Social Services Department and to long-term care in general.
  • Knowledge and ability to access community resources, i.e. Meals on Wheels, Home Health, Companion Services, etc.
  • Basic medical knowledge of the geriatric population living in long-term care facilities.
  • Ability to assess care level and needs of nursing facility residents.
