Job Description

Our Campus was one of the first to introduce the concept of continuing care and improve the quality of retirement living in North Central Phoenix.

We are currently looking for a Home Health Certified Nursing Aide to join our team!

The Home Health Certified Nursing Aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse and performs various personal care services as necessary to meet the patient’s needs. The home health aide is responsible for observing patients and reporting these observations and documenting observations and care performed. The home health aide will be assigned in a manner that promotes quality, continuity and safety of a patient’s care.

Home Health Aide Duties and Responsibilities:

  • Assist clients with ADL’s (Activities of Daily Living) i.e., dressing/undressing, dental care, bath functions, shaving, bowel and bladder functions, skin, hair and nail care( as per the home health aide plan of care), perineal care
  • Take and record oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
  • Change bed linens and keep clients immediate living areas clean and orderly, as appropriate per the Home Health Aide plan of care.
  • Assist in ambulation and exercise according to the plan of care
  • Perform range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
  • Assist with lifting/turning, moving, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. using gait belts as instructed and mechanical lifts as instructed
  • Meeting safety needs of patient and using equipment safely and properly
  • Report patient’s condition and significant changes to nursing supervisor/Case Manager
  • Adhere to organization’s documentation and care procedures and standards of personal and professional conduct
  • Maintain intake and output records as instructed
  • Collect specimens as instructed (urine, sputum, stools, etc.)

Skills / Requirements


  • Must have completed the CNA training program
  •  Possess and maintain current CPR certification
  • Successful graduation from an accredited CNA training program and certified through the AZ State Board of Nursing within four months of graduating from class
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
  • Knowledge and Abilities Required to Perform Essential Functions
